Your Care Coordinator will work with you to ensure that you get the most out of ONWARD. By providing personalized support and resources to align with what is reported on the actual ONWARD website, your Care Coordinator can help you:

Understand access and affordability

Your Care Coordinator will learn about your financial and insurance coverage. With this knowledge, your Care Coordinator can help you navigate the the insurance approval process and identify any potential financial assistance options that may be available to you.

Fit your treatment into your life

Because your Care Coordinator will become familiar with your lifestyle and unique needs, they can provide personalized resources to help you fit your new treatment regimen into your life. This could include checklists to keep you prepared for treatment day, reminders to help you stay on track, and other support resources for your everyday life.

Understand your treatment

As you have questions about your treatment, your Care Coordinator will provide resources to help you feel informed and confident.

ONWARD Care Coordinators do not provide medical advice and will refer you to your healthcare professional for any questions related to your treatment plan.

Advocate for yourself and communicate effectively with your doctor

It can be difficult to talk about your treatment and your condition. Your Care Coordinator can give you the tools you need to develop this skill, including communications strategies, connections to rare disease advocacy groups, and tips for self-advocacy.

YOUR CARE COORDINATOR CALL

After you’re enrolled in ONWARD, you’ll get a call from your Care Coordinator. This call is your first chance to get to know your Care Coordinator – and for your Care Coordinator to learn about you. Here are some tips you can use to get started on the right foot with your Care Coordinator.

Write down any questions beforehand
Take some time to think about any questions or concerns you may have for your Care Coordinator, before your call. Write these questions down so you remember to ask them during your call.

Treat it like a face-to-face appointment
Your first call with your Care Coordinator will be over the telephone. But it’s still an important appointment about your health. Make sure you set aside enough time and plan your day to comfortably fit the call in. If possible, find a quiet place where you can have your call uninterrupted.

Have something to take notes with
Your Care Coordinator may cover some new information during your call. To help you remember any new information, consider taking notes. When you sit down for your call, be ready with a pencil and a notebook or paper.

Bring a caregiver if you’d like
Like all of the resources available through ONWARD, your Care Coordinator can help you and your caregiver. If you have a caregiver who will be a big part of your treatment path, invite them to join the call.
